The College of Physicians and Surgeons has updated the Duplicate Prescription Pad order form, which is used by all prescribers. Dentists who require a new duplicate prescription pad should complete the order form on CDSBC’s website (not the version that is included within existing prescription pads) and send it by email to registration@cdsbc.org or by mail. We will continue to submit the form to the College of Physicians and Surgeons for processing on your behalf.
